They usually shim your ball aside, so you can't apply your muscles there.Or chemicals to make the area flaccid. Depends on where they need to cut.

shim my balls? i thought they where working on my eye.... oh eyeballs... right ok...I know they're doing eye drops, cutting open the gooey layer on my eyeball, removing the flower shaped bubble in my eye, then stitching it back up - eye scars!
