We had a guy at our company once
who had agreed with his line manager that he needed to work from home for a while because of family stuff. Line manager had left soon afterwards.
Two years later, a new head of his department came in, and noticed that they were still paying someone full salary and he'd not actually done any work for the last 20 months.
He was contacted and told he needed to come back to work if he wanted to carry on being paid and he said he was fine where he was and had wondered when they'd notice...

I took over a role from a half-wit that kept few records, didn't much care and had been sacked for gross incompetence.
Going through the budget I found there was a part-time member of staff employed by his predecessor (five years before) that no one knew about.
