and a mechanical arm (doing a pre-programmed motion without sensors to detect if some stupid piece of meat has just strolled in the way) doesn't sweep round and take his/her face off perhaps?

There's a system for working on machinery like this (lock out/Tag out) Where the machine must be rendered absolutely incapable of movement before the guards are taken down or someone goes to work on it. Human error*** pure and simple.
I have a bee in my bonnet about this sort of thing from my days working in a factory. I saw too many near misses caused by foremen ordering people to do really dangerous stuff, well outside their remit, just to 'get the job done'. No job's worth dying for.
*Supervisor/foreman/whoever ordered the work to be done
** Fired probably and/or jailed for wilful negligence
*** Hopefully it wasn't the guy that died going outside the rules just to get the job done quicker.

No I dont think so.
I am too hot and tired to find it right now but I know it was in Czechoslovakia and they had thousands of slaves working for them.
