One of my parents' neighbours many years ago refused to pay for dentistry.
When the pain of toothache was too much for him, he'd tie string round the tooth, tie the other end to a doorknob and then slam the door shut.

This was reputed to have happened several times. I have met witnesses.

I often walk past the old dairy where he worked back in the 60s, and I shudder. Dave Owen - the meanest milkman in town.
