This is a question No Self-Awareness

I had a boss who had no idea of his body odour problem, and everybody was too tactful to break it to him. Not so a visiting Rev Ian Paisley: "What the blazes is that smell? Is it you?" That sorted it. Stories of people blissfully unaware of their bad smells, bad manners and foghorn voices.

I too knew someone who did this
After 2 years of knowing him I pulled him up on it and it turned out he was partially deaf, he didn't like admitting it so he would lean in uncomfortably close when you were talking to him so he could hear, then subconsciously decided if he couldn't hear you from further away then that must be how close he needed to be to be audible when he replied.

This.
Someone I know is pretty deaf. He's not trying to make you uncomfortable when he leans in - he's just trying to hear what you're saying.
