OK, how it's done: JPEGs have an embedded thumbnail image that's not necessarily the same as the main image.

There's a neat bit of postcardware called Exifer you can use to view and edit the thumbnail and all the other embedded exif data.

The lady in the picture is Cat Schwartz (a TV technology presenter) who put the image on her blog after cropping out her naked boobs, however much to her embarrassment the cropping software left the thumbnail unaltered.
