These are police reported injuries/deaths. Given that most cyclists that ride into people (and I've seen this twice this week outside my office with deliveroo riders running red lights into people) just ride off after smashing into a pedestrian, what are you meant to report?

If cyclists are ace and no danger to anyone (which I'm sure the vast majority are ...just not the central London bike couriers), what's the problem with having a token licence (£1 a year), cycling proficiency pass and a number plate or you're not allowed on the road?

This is a normal post Does cycling proficiency still call for bikes to come to a standstill before pulling out around a parked car?
It was in my cycling proficiency (1979ish) but I've never seen a cyclist do that

They updated it a teensy bit something along the lines of "Give yourself more than a doors width clearance after checking that you are clear to pass".

That's from my 80s recollection though, so god knows if that's still the case.

This is a normal post I watched a YouTuber who records his journey
And that is what he said was the case. The vids were 2017 so it looks like it's the case. Not always possible of course

This is a normal post The problem is cars, even the Tories are beginning to realise that now
So when you discourage cycling you put more people into alternatives that already kill thousands a year
