AFAIK youtube vids always default to lowest quality, which has crappy-bitrate mono audio
The &fmt=18 forces it to use a higher quality, which looks a bit sharper but more importantly has good quality stereo audio. Although from a quick google it sounds like lots of videos no longer work with the &fmt=18 on the end :(
