So, to be paranoid, you'd fill this thing out from some wi-fi hotspot at a Starbuck's in a city you rarely go to. You'd connect to the Internet in an encrypted tunnel, with ssh or something, go through one or two anonymizing proxies and then fill the form out. And then, when finished, randomize your machine's disk with a mersenne twister and cleanly reinstall the operating system.
That should keep you reasonably safe.
