It was a humourous play on words, or 'pun'.Let me explain: 'fisting' could be taken as an alternative for punching, and 'ass' for donkey. Hence we arrive, by a somewhat convoluted route, at 'fisting in the ass', which as we all know, has entirely different connotations.
