Cocaine (the original local anaesthetic), also causes constriction of the them capilliaries, so no blood gets to them. That'll damage them. Plus there must be physical irritation of the nose membrane by the powder, and the user playing with it. Finally high blood pressure is one of the results of cocaine use; that gives poeple nosebleeds too-- look at 55 year old weegies.
