1) duplicate the layer,
2) shrink the bottom layer by half,
3) on the larger layer, cut large appendages into their own layer so each can be moved around
4) using warp or liquefy tool (this tool can be used for good sometimes) shrink the wrist/ankle/neck/whatever of the large appendage to match the diameter of the 1/2-sized part underneath.
5) position and erase to blend large appendage onto small body
