i believe it may be a case of excess skin...
and I suffered from something like that from being a fatass for a good portion of my life. I lost over 7 stone and had an unhealthy amount of 'excess skin'. solution? I got it all chopped off.
BUT it was never as bad as the WTF in the picture.
