Death at the gym
Almost killed some guy at the gym when I was distracted by a hot fitness instructor, and stopped to talk to her while removing the weights from the squat bar. She laughed along at my jokes while I removed all 60kgs from the one side of the bar only to watch it flip up on the rack, drop down 6 foot to the other side and almost spang some guy in the face on the bench press. Complete facepalm moment. He just got up and handed the bar to me as well, didn't say a word. In the reverse situation I would have knocked myself out for such stupidity.

Similar incident.
And it makes me cringe inwardly everytime I think about it some 16 years later.
I took the weights off one side of the bar whilst it was on a bench without sliding it to prevent it flipping over. The bar flipped (aided by about 50KG of unsupported weight on one side) and smacked right into the rowing machine opposite, which was luckily unoccupied.
Thinking of the damage it would have done had someone been using the rowing machine makes me feel very sick. I was very lucky to get away with that massive moment of stupidity. And I like to think I evened out the balance when I wrapped my car around a steel girder in a carpark years later during a similar moment of absent mindedness. :)
