Expensive Mistakes
coopsweb asks "What's the most expensive mistake you've ever made? Should I mention a certain employee who caused 4 hours worth of delays in Central London and got his company fined £500k?"
No points for stories about the time you had a few and thought it'd be a good idea to wrap your car around a bollard. Or replies consisting of "my wife".

Hiss, rumble, BANG!
When I started out in haulage, I had an HGV licence, and fuck all knowledge of how a 38 tonne truck actually works. One morning, I hooked up a trailer. To do this, you attach a red airline (emergency supply) a yellow airline (brake), primary and secondary electric and the ABS lead. Until the yellow line is attached, there is NO trailer brake, so the tractor unit handbrake MUST be on. I know this now, because the whole outfit rolled across the yard and totalled the cab. £15000.
That was 18 years ago, and people still do it. Two in our yard in the last 3 months, and lucky they didn't die.

bloody hell!
My dad was a haulier when I was a nipper and some of the stuff he told me about things that can potentially go wrong with a 38 tonne arctic make your hair curl. While not an expensive mistake, I remember the day he was roping and sheeting some pallets and putting his whole body weight on the rope to tighten it. The rope gave way and flew backwards, landing on his outstretched arm which snapped like a twig. His comment after was "i was a bit bloody shocked, but not as shocked as the guy in the P and O lorry who was just pulling out behind me and had to swerve to avoid my head...". I've always had a lot of respect for trucks and truckers since!
