The local PCTs have a budget to balanceand they're the ones who cheerfully stick you on a cheaper alternative. NICE only comes into play on the issue of licensing a treatment or drug for use in the NHS as a whole.
The fuckwittery I've experienced for example, is a purely local issue.,
