The company I work for, when issuing an apology stemming from a racial discrimination lawsuit, used the phrase "should of."

It's definitely the worst thing anyone can do.
How the fuck can people do that and not notice how weird it is? A modal followed by a preposition. Some people should be shot.

The only
thing that could have made it worse would have been the use of the word 'loose' in place of 'lose'. Every time I see it, it makes me wonder what they do when they really want the word 'loose'. 'Looose' perhaps?
