Not the face so much,more the belly button up where most people's diaphragms are, disproportionately wide shoulders and massive hand, and his giant head with different lighting.
Having said all this it'll probably turn out to be a real photo after all :(

It may well be just an awful bit of photographyI'm not sure how the photographer managed to miss the fact that his left hand makes him look like he is fully tumescent
