
The foil bowl piece should attach to the biro case (which enters the water) and the mouth should go over the bottle top to inhale the water-cooled smoke.
As it stands, the crack is just incense and the biro is a bong-water drinking straw. Worse than useless.

It is more efficient to use the biro as the draught hole/shank than as the stem/bit/button. The large mouthpiece leading to a narrow draught hole would create a more powerful suction than the inverse.

It is no different to putting the foil directly onto the pen part, if all areas are sealed it works the same and no need for water!!

Suppose we apply negative pressure to the biro mouthpiece as illustrated above (assuming the bottle is waterless). Both the bottle and the bowl are significantly greater volume than the draught hole resulting in a longer, but weaker burn; you would need to suck a lot to get a little. You've created, ironically, a bottleneck.
Reverse the setup so the bowl is mounted on the biro and the bottle opening is the mouthpiece. Now you only need to suck a little to get a lot. 1 standard toke results in a lungful, rather than a gnat's fart; there's a higher delta-p at the bowl and a much faster transfer rate from chamber to mouth to lungs.

A better design would probably do away with the plastic parts completely (except for using the biro as a mould for the foil, possibly), but I digress.
