hmm. I do it from scratchbut I do have the photo either in a layer or in another window. It's hard to describe. I made a video of how I do it but it's fucking massive right now and needs to be sorted and tuned up for youtube.
Edit. it's not done in photoshop btw. Corel Painter for it's seriously good way of managing pixels like paint.
