He's trying to characterise how crazy he thinks ThE LeFt are.
To him, the very idea that someone can legally change their sex and that decent people will politely accept using preferred pronouns is as ridiculous as the idea that when men vote for women, some form of transsexual transubstantiation event occurs.
Also real men don't vote for yucky, icky women.
