It could actually be a guy who's into injecting silicone into his balls to make them huge. It's a thing, you can find it online.

This is a normal post I wondered that too
recently been chattin to a bloke that does it with saline, as you do, I imagine it could be easy to slip and need hospital attention, then what story would you tell...

*edit* just in case anyone reading this is stupid enough to try it, I should perhaps mention, you inject the ball SAC, not the actual balls, which would kill them and probably you.

You're right, probably saline rather than silicone. The thing with saline is that your body reabsorbs it... so even if you had injected loads and then the needle broke you could just wait an hour or two before going to the hospital.
