When a boat my friend was on sankher purse went down with it, including her driving licence. Filling in the form for a new one it had the same options - it wasn't stolen, except by the sea, it wasn't lost - just couldn't get to it and it probably wasn't damaged!

Brilliantin the defence of the bank it would be hard to cover all possible card related instances.
I would have just put it down as lost, even tho she knew where it was..
