In the case of breast implants, a pocket is made in the breast, and the implant slipped in - the nipple is not disturbed.

In the case of disease, or radical reduction, the nipple is carefully dissected leaving the nerve and blood supply intact, then replaced, otherwise feeling is lost, and it can drop off.
