I suffer the exact same thing!It even goes to the extent that when at work and you go for a widdle, you suddenly imagine yourself standing by your desk doing it in front of everyone then get paranoid incase you actually are (thankfully hasn't happened!!!).
Or when you're walking through with the tea round and everyone looks up and you think -my god did I just say something or is my knob hanging out? When actually they just want tea!
Worst was my ex, I used to call her a pet name as whenever I went to say her name I always had my previous ex's name on the tip of my tongue (let that one slip once and vowed never again!)...
