translation: The poster, a 'techie' was not profficient in his job enough to prevent users from making changes far above his station. A savvy user abused this position to play a prank which the poster then had to fix. Had the poster done his job properly, this would never have happened.

First: most techies in an IT office will not be expected, or permitted, to fiddle with policies.
Second: there are so many policies, you're bound to overlook one or two minor cosmetic ones, until they come up.
I conclude: you've not set up any servers in recent memory.
