If someone's 'been down' for 20 minutes without breathing, one way or another, they've probably had a cardiac arrest.
And if someone's been fuckwitted enough NOT to intubate someone after 20 minutes of not breathing then, no matter how good your resus skilz are, they are probably, in the long term, to use medical parlance, fucked.

More to the point, if they've been intubated, then they're probably not heavily sedated and/or paralysed and very unlikely to breathe..
