A friend was once given a biopsy by a sleep-deprived junior doctor.
They needed a sample of his colon, so inserted the long bendy jaws-on-the-end thingy, located the suspect area and... he shot through the ceiling. Doctor had forgotten to administer any anaesthetic.
What was your ouchiest moment?

My parents were given advice about dealing with difficult children, back when me and my brothers were starting primary school. Things like 'when they do something right, praise them a lot, so they get used to liking doing things right'.
Fifteen years later, they've long got into the habit, and we still get a Great Big Smile and an effusive 'Well done! You're doing really well' whenever we empty the dishwasher. Drives you mad. MAD I tell you!
