Unless you are in a 3rd world countrynot many people on building sites get killed because they violate rules. Most of the deaths (which aren't many)* are outright stupidity (Violating laws of physics) , that sort of shit OH&S will never cure.
*not many depends on your definition-some people say one death is too many, while I think that the world is overpopulated and should be allowed to sort itself out.
