I've had a play with it.
An espresso shot (or if using instant, only just enough boiling water to dissolve the coffee and sugar) in a cup - the cup drains a lot of the heat out of it, which then isn't transferred to the final drink. Then the shot is poured slowly over three or four ice cubes in a glass and swirled until it feels cool. Then add very cold milk. If adding icecream, roll it into a ball a little less than the width of the glass and carefull add it before the milk. Pour the milk in slowly onto the icecream to give a neat layering effect with the coffee at the bottom and milk on top.
