spreadshirt might be ok, although they may refuse the design on copyright grounds (although, if it's not a vector, they probably won't check)
other than those couple, i've found the returns don't ever seem to be worth the effort, and the end user price always feel extortionate.

cafepress were MASSIVE cunts

edit: rob set up a tshirt shop a while ago with 'shirt of the week' type things, gaz him to see if that's a goer

I've done a couple of things with speardshirt but it limits the colour of shirt you can use. Good for vectors though limited to three colours.
In MySoti's defence the result I've seen was very good.
