Only other explination I can think of is interlaced footage, getting the field order wrong on import then a low bitrate encoding at some point.
But I can't see it anwyhere else

My money is on a straightforward encoding artefact
The face in the background is relatively low brightness, and I've seen many cases where low brightness areas show up encoding artefacts - having a large high brightness hand move across the low brightness face could be an example of this.

It's more likely that guy actually has six fingers on his right hand, and they decided to do a quick edit, so people wouldn't think it was AI generated
