I know how elements of this are done but the actual disappear is just a little bit baffling.

looks like
at 1:36, either he switches to some fake card, or when he slides it into the pack,there's something weird going on there. Hard to tell at 3 pixels by 3 pixels though.

He's using a card box called Tool (by a guy called David Stone)
It has a removable magnetised face on one side which is the shape of a card, so you can hide cards within it.
Not sure where the blue card within the two jacks goes though. Probably folds onto the back of the top jack or something.
