A blood clot is not, usually, something you'd cause a vagina by entering and I doubt a clot in the vagina, in the sense you seem to be referring to, would even happen.
I take it you mean she had a clot in her lung, which she coughed up due to heavy breathing?

I'm not even going to pretend that I'm a doctor
but she did have a clot. I think the doctors finally worked out she had a preexisting condition that caused it. However, it chose the wrong moment to arise (excuse the pun)

As for the blood, I presume it was from when she stood up or from moving back and forth. It was pitch black so I couldn't really see
