The whole room falls silent, apparently, at this formal meal she'd just said penis, quite loudly...

It does indeed mean cock. I work in kindergartens so hear it quite a bit, but we also hear it out and about when getting drunk. Usually from ourselves when we try and excourage a rousing shout of "chin chin" from the locals. Normally goes down quite well, but there have been the occasional dirty looks from salarymen who haven't had enough to drink yet!

The South Park episode (s3e10) titled 'Chinpokomon' went right over my head, till I watched it with Japanese friends having been in Japan for a few months, and they pointed out all the subtle (wtf?) jokes that had been peppered throughout. Trey and Matt obviously had someone working on the inside, as there's a lot of piss-taking from the Japanese side.

To be fair, shouting 'cock' at a formal meal is good banter no matter where you do it
