@SpakkamanI had to do that last year too.
In the space where it asked why I was off work, I simply put 'diarrhoea'.
In the space where it asked for symptoms, I put 'watery brown arse gravy pouring uncontrollably from my poor, suffering sphincter'
Strangely, my boss didn't ask me for further details on that one.
