This is a question Ouch!

A friend was once given a biopsy by a sleep-deprived junior doctor.
They needed a sample of his colon, so inserted the long bendy jaws-on-the-end thingy, located the suspect area and... he shot through the ceiling. Doctor had forgotten to administer any anaesthetic.

What was your ouchiest moment?

Tyres
Many moons ago I saw an article about similar incident in an HSE pamphlet when a lorry tyre burst killing the tyre fitter. The article was illustrated with a photo of a dusty ceiling with a man shaped clean bit.

it still happens on site
last year we had a tyre fitter in the yard changing the tyres on the telehandler
he pushed one under his van to seat the bead and had a rim failure and judging by what it did to the underside of the van it would be fatel to be any where near when it happened.
