Why wasn't he replaced as leader of the Decepticons ages ago? Repeated failure after failure to destroy Optimus Prime and the rest of the Autobots, he should have been given the sack years ago.

It depends which continuity you follow
Cartoon Megatron spent two seasons being utterly useless before returning in the animated movie and fatally wounding Optimus Prime in one-on-one combat before being betrayed by Starscream. Then he was reformatted into Galvatron, killed Starscream and took control again, but was ultimately defeated and driven mad, after which he spent another season being utterly useless.

Marvel Comics Megatron was similarly useless but at least Shockwave spotted that, beat him down like a little bitch and took command. He eventually ceded command back to Megatron, but after Megs went crazy and blew himself up in the space bridge Ratbat took over as leader, and he was actually pretty efficient and good at the job. Decepticon leadership then generally alternated between Thunderwing on Cybertron and Scorponok on Earth, both quite competent in their own right, and then Bludgeon after Scorponok's demise. Even when Megatron came back he wasn't in charge of anything, just a loony in a base with a handful of followers.
