so i'm only commenting on the thumb. I'd blend the fish bit with a gradient mask right up to her belly button. but other than that...nice

Is the gradient mask where you click "new layer mask" an then draw on it ith varying shades of grey to show through the layer underneath? I've not quite got the hang of layer masks just yet...

just use the gradient tool (same place as the fill tool) on the layer mask

and the varying shades of grey is the gradient fill tool...in fact, towards the very top of the fish bit i'd have a layer of scales bump mapped (displaced) slightly under the womans skin so it looked like fleshy scales, then fading into the fish with the layer mask.
