I think it's mostly the States where things are this dire.
A combination of harsh sentencing laws that give it a staggering incarceration rate, privatised services where they want more prisoners but want to spend as little as possible, and a general kind of punitive "fuck 'em" attitude in the culture, which keeps the cycle going. Not that it's a doddle in other countries

About guards in a Russian prison organising systematic and ritualistic rapes of inmates. They’d do it themselves or use broom handles. And they filmed it. It was fucking bleak.
