It's a little like teaching stats
If you have dumb kids to teach/sicker patients your results can be worse through no fault of your own, it's not as simple as the politicos like to make out. Yes you need to monitor people's performance but not with a blunt instrument.
Increased death rates in some trusts are more likely to do with cost-cutting and outsourcing to the lowest bidder while employing more managers than nurses.
