CackersKuru is a prion disease, like nvCJD, caused by a mutant protein in the brain. In the same way that eating neural tissue from a BSE-infected cow can give you CJD, eating neural tissue from a kuru-infected person gives you kuru. Hence why it's confined to a cannibal tribe.
