I once cut the power to and blew the hatch on a mixing vessel with a broken thermostat that was over 150°C inside (it wasn't meant to go higher than 85.)
The batch was ruined and there was was an appalling stink of burnt fats lingering round that area for most of the shift, but there were no injuries or damage.
I'm sure many jumped-up Little Englanders would wind themselves up into a fit of impotent rage at the thought of being legally obliged to investigate why this happened, get the thermostat repaired as a result and have the whole process documented. Having to put up with preventable industrial accidents was good enough for folk a hundred years ago, why can't it be good enough now? Facking lefty PC brigade. Hanging's too good for 'em, the facking cants.
