Actually the Rosetta spacecraft fell foul of ESA co-operation - the german software completely overwrote the Belgium operating system causing the French instrumentation suite to automatically shut down.
With no operational satellite ESA faked the whole mission by using a pebble from the beach at Noordwijk on a turntable filmed by a suspended iPhone with the lights off.

People have died for telling this true story........
