Why should we have to wait at all?
The amount of work is precisely the same whether the patient has waited thirty seconds or four hours. Or maybe they just hope that a significant proportion will die before treatment. It works for hip replacements, after all.

by appointment only? Approved injuries only, I suppose?

Edit: on rereading, have you mixed up A&E, GP appointments, and Elective surgery?
