As long as they only do it to themselves then there is no harm. I think that changing your body that way is just the same as getting a boob job or a nose job.

You wouldn't say that they were mentally disturbed. To adorn or change your body is something that is deep within us and for some of us Western people, needs to be realised.

when people feel the need to mutilate their genitals, especially the woman who has had her clit and labia taken off, must have some deep seated issues with their own sexuality.

Most people go into their modifications with a clear and intellectual idea of this is how they want to be. People who do modifications to themselves are only doing it because they cannot find doctors who will do it for them.
