If they had any clinical reason to query the dose, then yes they are.
I've got lots of lolprescribing error stories, like the person who'd been getting meloxicam 15mg twice daily for until I queried it, and it turned out the hospital letter said "double the dose from 7.5mg daily to twice daily" and they quadrupled it by mistake. Without anyone apparently noticing that this was an overdose, for over two years.
