and now that medical science is such that these procedures can be done relatively safely and life is short, I imagine the main thought would have been

but this is body modding, there not allowed by law to use anesthetic so he had that done, his nose cut up and a big bit of cartilidge removed WITHOUT MEDICATION!

Imagine how much that would have hurt! and for what? so everyone you meet can say "wow you look like a klingon!" what a nob!

Oh, and from the look of what went on, it doesn't look all that painful. A few hundred years ago, people had their limbs removed without anaesthetic and somehow got through the pain. Women give birth all the time without pain relief. If people weren't such wimps nowadays it wouldn't seem like such a painful thing to do.
