You couldn't
"You couldn't pass me that paper?"
"You couldn't do me a favour?"
You couldn't just bloody say 'Could you', or even 'Please could you' if you are worried that sounds too forward...
When someone asks with "you couldn't" I just want to say "That's right".

I'm not defending the practice,
however I think it's intended to convey humility. A sort of shorthand for "I've no real right to ask, but". Of course it's a false humility as none of us ask this type of question if we don't think that there's a good chance of an answer in the affirmative.

Not to mention the mis-use, and probably misunderstanding of,
the difference between 'would','could' and 'should'.
"Would you ..." and "could you ..." are two quite different things.
