Pardon or Excuse me...
If you didnt hear what I said then just say either "pardon" or "excuse me"....
Saying "say again" just pisses me off to the point where I want to tear off your ears and skull fuck you....
That and people singing the wrong lyrics to songs....

Having spent some time in the army cadets I can confirm it's a military thing
If you didn't catch what someone said to you over a radio, the correct response is "Say again, over." This is (apparently) because "Repeat" is an artillery command and if you say that you might get mortared by mistake.
So yeah, I say "say again" mostly out of habit from those days, especially if I'm on the phone. Sorry, I didn't realise it was annoying.

"Pardon" or "Excuse me" both suggest an error on the part of the speaker
And sometimes it's the other person's fault for mumbling or speaking quietly. "What did you say?" would be much more even handed.
